Note: In order to complete this application, you are required to submit the items listed below, if you have a legible scanned or photographed digital copy, you have the option to upload them as part of this online form:

1. Certificate of Insurance showing Comprehensive General Liability Insurance, including but not limited to: bodily injury, property damage, personal injury, and broad contractual liability, in the amount of not less than Five Million Dollars ($5,000,000).

2. Such insurance shall name the Municipality of Trent Hills as an additional insured, and shall include a provision or an endorsement that the Municipality of Trent Hills will be provided a minimum of Ten (10) days notice in writing of any cancellation, expiration or variation of the policy.

3. Site plan showing location of tables, chairs and umbrellas or retail displays with measurements to show space available for pedestrian passage and physical distancing.  If using umbrellas please include height and width.

If you do not upload the required documentation, you will need to provide a hardcopy of the documentation at the Municipal Office.

By clicking "Submit", I Acknowledge if a Temporary Sidewalk Use Permit is granted that:         

1. The Applicant/Business shall assume all liability arising out of the operation of the business activities on Municipal property (sidewalk/parking spot) and agrees to indemnify and hold harmless The Corporation of the Municipality of Trent Hills, its agents, servants, employees and officials from any claims, actions or suits which might be brought against the said operation in any manner whatsoever or any error, negligence or omission of the operator, their agents, servants or employees. 
2. The Applicant/Business has reviewed and agrees to any regulations stipulated by any applicable Municipality of Trent Hills bylaws and policies. 
3. The Temporary Sidewalk Use program runs from May 1st until October 15th, 2021
4. Failure to comply with approved site plan will result in the revocation of permit and 24 hours to dismantle.
